Questions & Answers

Do I need to bring anything?

Please bring your photo ID or license and social security card. If you have lost one or waiting for a new one to come in the mail, still come to orientation. You can bring us a copy before the first day of class.

I am under the age of 18. Do I need to do anything special?

Yes! You must get a Minor Release Form filled out. You can find it at: https://www.forsythtech.edu/courses-programs/job-skills/college-career-readiness/adult-high-school/ at the bottom of the page or you can get one at registration. This needs to be filled out by the student, most recent high school, and it requires a notarized signature of a parent or guardian. 

You must also complete the Leon's Law Form. You can find it at: https://short.forsythtech.edu/FTLeonsLaw 

I want to do the Adult High School Program

Great! We will need a copy of your offical transcript from your most recent high school. You must have 15 passing credits from high school to enter this program. You can have the transcript sent to Susan Towe at stowe@forsythtech.edu
