You must turn in a completed Minor Release Form. You can find it https://www.forsythtech.edu/courses-programs/job-skills/college-career-readiness/adult-high-school/ at the bottom of the page. This needs to be filled out by the student, most recent high school, and it requires a notarized signature of a parent or guardian. 

Please bring your photo ID or license and social security card. If you have lost one or waiting for a new one to come in the mail, still come to orientation. 

Please plan to be here for up to 3 hours. This allows time to fill out paperwork and complete your placement test. 

Great! We will need a copy of your transcript from your most recent high school. You must have 15 passing credits from high school to enter this program.
